Indications

• Treatment of inflammatory skin conditions that are complicated by bacterial or fungal infections. Conditions such as eczema, dermatitis, psoriasis, and certain types of rashes that are infected or at risk of infection.

Mechanism of Action

• Corticosteroid (e.g., Betamethasone): Reduces inflammation by inhibiting the release of inflammatory mediators.
• Antifungal (e.g., Clotrimazole): Inhibits the growth of fungi by altering the permeability of their cell membranes.
• Antibacterial (e.g., Neomycin): Inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit.

Pharmacokinetics

• Absorption: Topical corticosteroids can be absorbed through the skin. The extent of absorption depends on the use of occlusive dressings, the integrity of the epidermal barrier, and the specific formulation.
• Distribution: Primarily local at the site of application; minimal systemic distribution unless large areas are treated or occlusive dressings are used.

Contraindications and Precautions

• Contraindications: Hypersensitivity to any of the components (betamethasone, clotrimazole, neomycin). Not to be used on untreated bacterial, viral, or fungal skin infections. Avoid use on large areas of damaged skin or in the presence of occlusive dressings.
• Precautions: Use with caution in patients with a history of local hypersensitivity reactions. Avoid prolonged use, especially on the face, or on large areas of the body, or with occlusive dressings, as it increases the risk of systemic absorption and side effects. Not recommended for use in nursing mothers unless necessary.

Adverse Reactions and Side Effects

• Common: Burning, itching, irritation, dryness, redness of the skin.
• Serious: Skin atrophy, striae, telangiectasia, perioral dermatitis, allergic contact dermatitis, secondary infection. Prolonged use can lead to systemic effects like hyperglycemia, glucosuria, Cushing's syndrome.

Interactions

• Drug-Drug: No significant interactions for topical use, but systemic absorption can interact with other corticosteroids or immunosuppressive drugs.
• Food: No significant interactions.

Route/Dosage

• Topical (Cream):
