Indications

  • Relief of coughs due to minor throat and bronchial irritation as may occur with the common cold or inhaled irritants.

Mechanism of Action

  • Dextromethorphan: Suppresses the cough reflex by direct action on the cough center in the medulla.
  • Guaifenesin: Acts as an expectorant by increasing the volume and reducing the viscosity of secretions in the trachea and bronchi, facilitating the removal of mucus.
  • Phenylephrine: Acts as a decongestant by constricting blood vessels in the nasal passages, reducing swelling and congestion.

Contraindications and Precautions

  • Contraindications: Hypersensitivity to any component of the formulation. Concurrent use of MAO inhibitors or within 14 days of stopping such treatment. Severe hypertension, severe coronary artery disease.
  • Precautions: Use with caution in patients with high blood pressure, heart disease, diabetes, thyroid disorders, enlarged prostate, or glaucoma. Avoid use in pregnant and breastfeeding women unless prescribed by a healthcare provider.

Adverse Reactions and Side Effects

  • Common: Drowsiness, dizziness, nervousness, nausea, vomiting, stomach upset.
  • Serious: High blood pressure, rapid heartbeat, palpitations, confusion, hallucinations, severe allergic reactions (rash, itching, swelling, severe dizziness, difficulty breathing).

Interactions

  • Drug-Drug: Increased risk of hypertensive crisis with MAO inhibitors. May interact with other central nervous system depressants, antihypertensive drugs, and certain antidepressants.
  • Food: No significant interactions, but avoid alcohol due to the risk of increased drowsiness and dizziness.

Route/Dosage

  • Oral (Syrup):
    • Adults and children over 12 years: Typically, 10 ml every 4-6 hours as needed, not exceeding 60 ml in 24 hours.
    • Children 6-12 years: Typically, 5 ml every 4-6 hours as needed, not exceeding 30 ml in 24 hours.
    • Children under 6 years: Dosage should be determined by a healthcare professional.
